Service Description

Evaluation: 60 minutes - Discuss goals : We will get on the same page about all of your goals for health and recovery. - Assess impairments : Professional assessments will be performed in order to gather data on you physical health and condition. - Diagnose problem : A diagnosis will be determined or confirmed through tests and measures. - Create a plan: A solid plan of care will be established. - Documentation provided: All findings from the evaluation will be presented in a hand out at the end of the session.

Cancellation Policy

Cancelations and rescheduling an appointment must be completed 24 hours or more in advance or you will be charged for the appointment.
